A restructure within the Aboriginal Chronic Care Team is being proposed by HNELHD. This is your opportunity to have a say on how this affects you.
- Management claim this proposal is needed to realign the current Aboriginal Chronic Care initiative within an Integrated Care framework, which they suggest would strengthening both strategic and operational leadership.
- The proposal would see various changes to reporting lines and also change the title of some positions.
- The impacted positions are the Aboriginal Program Lead and the Aboriginal Chronic Care Project Officer.
